Schedule: Monday - Friday (8:00am - 4:30pm)

MINIMUM QUALIFICATIONS (REQUIREMENTS)

Required: Education and experience sufficient to succeed, carry out responsibilities, and demonstrate the skills and abilities as described above with at least two yearsâ experience working in pre-hospital or post-hospital setting. Extensive experience working with individuals experiencing mental health and addictions issues is preferred. Relevant experience includes: Housing Placement and Retention work Social work or behavioral healthcare work Case management and/or care coordination work Substance use disorder treatment experience Required: Minimum of two (2) years experience working in low-income housing or similar setting.

SUMMARY: Evergreen Crossing is an innovative program that will offer integrated housing, health care, and employment services with the long term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations. This position will work as a member of a multi-disciplinary team at Evergreen Crossing to deliver comprehensive, person-centered, integrated services to the Portland Eastside community.

The Housing Specialist works as a member of a multi-disciplinary team to assist participants in the Recuperative Care Program with identifying, securing, and moving into affordable permanent housing, transitional or other placement options when they are discharged from RCP. RCP works with partner hospitals and referring agencies to provide recuperative care for both the acute and ongoing medical needs of patients and also providing short-term emergency supportive housing, crisis management, referral to appropriate substance abuse, physical health, and mental health treatment, access to benefits and entitlements, appropriate and affordable housing, and further provide solutions to ending-homelessness and achieving self-sufficiency. The Housing Specialist coordinates with case management staff and in collaboration with community partnerships on the development and execution of housing and treatment plans.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Work as a member of a multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services. Actively participate in the integration of services at Evergreen Crossing and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines. Proactively identify opportunities for improvement within Evergreen Crossing workflows and logistics. Participate in review of Key Performance Indicators; review and contribute to continuous improvement opportunities and plans. Build understanding of and appreciation for diversity among Evergreen Crossing clients and support their connection to culturally responsive and culturally specific services as requested. Pursue affordable housing options RCP program participants both inside and outside the Central City Concern system. Work with other Recuperative Care and Evergreen Crossing staff to support the appropriate and effective use of primary care and specialty follow-up care. Assist program participants in completing all appropriate forms for obtaining housing. Attend housing and placement-related meetings required in order to best work with landlords, resident services and building staff. Assist program participants through development of relationships with landlords, resident services & building staff. Track all necessary documents such as housing applications, lease agreements and other housing forms for RCP patients. Provide support to some program participants placed in the community as needed for a period of time post-RCP. Advocate for program participants when necessary; attending meetings such as landlord/tenant meetings or court. Become knowledgeable of landlord tenant law, Shelter-Plus Care voucher system, Section-Eight, and any other programs as deemed necessary. Help patients move, secure furnishings and supplies, and/or make improvements to their dwelling to support housing stability. Cross train with other staff to provide relief for breaks and time off. Complete all paperwork and electronic records accurately and in a timely manner. Attend all mandatory CCC trainings in a timely manner. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partnersâ and outside providersâ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. Attend all scheduled staff meetings and staff trainings. Participate in training opportunities to advance skills and knowledge in addictions and mental health. Develop relationships and community resources as appropriate. Perform other duties as assigned.
