Position Title: Dementia Care Coordinator - CNA

Hours:  Monday - Friday, 8am - 5pm

Overview:  The Dementia Care Coordinator - CNA will work with dementia patients receiving hospice care at home and in facilities to find activities that engage the interest of persons with Alzheimer's disease and other dementias. The coordinator models activities to family caregivers and facility staff. The coordinator will provide relevant education identified as a need by caregivers and staff including but not limited to strategies for bathing, engaging in meaningful activities, and redirection to maximize the comfort of both the person with dementia and their caregiver(s). Additionally, the Coordinator will function as an IDT member for the patient, providing information to other care team members during IDT and individually, as appropriate.

You Have:

  • Current Florida CNA license, required. High School diploma or equivalent, preferred
  • One (1) to two (2) years CNA experience. Experience with interdisciplinary team, preferred
  • Ability to embrace individuals from all walks of life

Location: Orange Park

The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area.  The care center is located at 745 Blanding Blvd, Orange Park.
