ResponsibilitiesAs a member of the Central Assessment Team, the Inpatient Patient Intake Coordinator, housed within the Brooklyn Park hospital, is responsible for coordinating admissions into the hospital level of care.
Responsibilities and Accountabilities:
- Review and link callers with information about our services
- Triage crisis calls
- Facilitate internal referrals for inpatient services
- Facilitate external referrals for inpatient services
- Consult with physicians and nurses as needed to facilitate referrals into our inpatient services
- Provide effective customer relations pursuant to the policies of PrairieCare
- Provide external referral resources as needed
Hours: Friday, Saturday & Sunday 8:00am - 8:30pm
Qualifications- Requires a Bachelor's degree in Social Work, Sociology, Psychology or a closely related human services field
- Two years of supervised experience in behavioral healthcare is required, meeting the criteria for a mental health practitioner, or a license as a Licensed Social Worker (LSW)
- Experience in triage of psychiatric emergencies is preferred
- Possess interviewing skills and display a professional attitude in all aspects of his/her performance
- Oriented and educated in gender, cultural differences and in age appropriate care for psychiatric and chemical dependency services
- Must possess skills to communicate clearly and effectively with clinicians
- Must have computer skills using Microsoft Office Products including Word and Outlook
- Ability to smoothly and rapidly shift between dissimilar tasks
- Ability to communicate well both verbally and in writing
- Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
- Ability to make effective decisions while in stressful situations and circumstances
- After training, be able to manage disruptive and explosive patients.
