SUMMARY
The LPN Team Case Assistant will utilize clinical experience, and coordinate patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.

ATTRIBUTES
Graduate of an accredited school of licensed practical nursing, required
A valid NYS license as a Licensed Practical Nurse
Proficient use of Microsoft Office
One year of experience in home care setting or medical office preferred
RESPONSIBILITIES
Customer Service. Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers' needs are met. Validates supplemental orders and processes per policy.
Supports Field Staff. Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee. Remains available to assist in other duties deemed appropriate to the scope of practice of an LPN.
Program Review. Collects, organizes and reviews various programs for information and reporting and initiates follow up.
Discharges. Performs functions related to transfers or discharges as required by regulation.
Clerical Work. Performs general office duties/projects including but not limited answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.
Orientation. Assists in orientation of new staff, role modeling, participates in quality assurance activities.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

14513The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
