We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, providing medical support, and ensuring the best possible outcomes for our patients.
*Duties*
- Utilize physiology knowledge to assess and monitor patient conditions
- Manage critical care cases in both inpatient and outpatient settings
- Monitor vital signs and respond to any changes promptly
- Collaborate with interdisciplinary teams to provide comprehensive care
- Implement case management strategies to optimize patient health outcomes
- Specialize in areas such as behavioral health, hospital medicine, and infectious disease care
- Work in a Level I Trauma Center environment
*Requirements*
- Valid RN license and BSN degree
- Proven experience in case management and healthcare coordination
- Strong understanding of inpatient and outpatient care processes
- Familiarity with infectious disease protocols and treatments
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
Job Type: Contract
Pay: $2,680.00 per week
