Role Summary:
The Clinical Case Manager plays a crucial role in delivering comprehensive care through effective screening, assessment, planning, and resource management. This position involves collaborating with patients and their families to address healthcare needs and facilitate smooth discharge planning. The Case Manager is an integral member of the interdisciplinary healthcare team, working closely with other professionals to assess patient needs and develop a holistic care plan.

- Facilitates a coordinated approach to care by integrating clinical and psychosocial strategies throughout the care continuum.
- Contributes to the planning, coordination, and execution of clinical reviews, discharge planning, and resource utilization.

Qualifications:
- Bachelor's Degree in Nursing or equivalent education/experience.
- Master's Degree in Nursing or a related healthcare field is preferred.
- Registered Nurse with significant experience may be exempt from obtaining a BSN.
- Compliance with any previously established agreements regarding educational requirements will be honored.
- Collective bargaining agreements regarding educational timelines will take precedence.
- Exceptions may be considered on an individual basis by senior nursing leadership in collaboration with Human Resources.
- RNs employed prior to specified dates are encouraged to pursue further education, though it is not a condition of employment.
- Must possess a valid California Registered Nurse License upon hire.
- Minimum of 2 years of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.).
- At least 2 years of experience in Case Management or successful completion of the Transitions in Practice (TIP) program for Care Managers.
