JOB SUMMARY AND QUALIFICATIONS:
The Intake Coordinator will assist with services to individuals and families experiencing a housing crisis in the Lincoln Trail and KIPDA regions. In tandem with other members of the VOA Home team, the Intake Coordinator will assist clients in enrolling into programs and determine eligibility. The intake worker will be responsible for entering data into HMIS and keeping log of client contacts.
WHAT YOU SHOULD HAVE FOR THIS ROLE:
Must have knowledge of customer service skills; previous telephone system experience and proficient computer skills; well organized, self-starter, and able to work as a member of a team. Current valid Driver’s License and reliable transportation.
RESPONSIBILITIES:
1) Determine the eligibility of those who seek assistance by completing the screening forms during the initial interview;
2) Process and prepare preliminary assessments of all very low income families referred to the KHC RRH Program;
3) Make appropriate referrals to all community partners and resource providers for crisis intervention and short term housing stabilization;
4) complete a screening over the phone and an assign eligible participants to the applicable case manager;
5) Enter enrolled clients into the Homeless Management Information System (HMIS);
6) Update and maintain all Excel spreadsheets or client trackers.
