As an active member of the Support & Services at Home (SASH) team, The SASH Coordinator will provide coordination of care and transitions of care for SASH program participants living in affordable housing. The SASH Coordinator will build trusting relationships with residents in order to develop a thorough knowledge of each SASH participant's strengths and challenges as they pertain to living safely at home. The SASH Coordinator collaborates with the on-site SASH team, housing staff, community partners, and the medical community on a daily basis to coordinate care and services to meet the needs of SASH participants. The SASH Coordinator is an active member of the Support & Services at Home (SASH) team as is expected to work collaboratively with the SASH Wellness Nurse (RN) in providing whole-person, care coordination and support targeted to individual participants, as well as the SASH population as a whole. Specific interventions by the SASH coordinator will be adaptable to meet the needs of program participants as it pertains to the prevention and management of chronic conditions.
Qualifications for the SASH Coordinator:
- Bilingual English/Spanish Proficiency.
- Must possess an Associate's degree or Bachelor's degree in social work, community health or equivalent combination of background and experience.
- Previous experience coordinating services specifically for the older adults and/or people with disabilities is preferred.
- Knowledge of area resources and programs available to older adults and people with disabilities (e.g., transportation, health services, recreational and wellness activities) is essential and, therefore, strongly preferred.
- Demonstrates basic computer competence and the ability to learn all relevant applications (i.e., electronic health records, care-management systems, spreadsheets, email and office software).
- Demonstrated excellence in verbal and written communication is required.
- Must possess a valid driver's license.
- A working knowledge of Microsoft Word, Outlook and statistical databases in general, is highly desirable.
Essential Responsibilities for a SASH Coordinator:
- Educates and informs SASH participants and family/support persons of the SASH enrollment protocols, which includes an assessment completed with the program participant upon enrollment and annually thereafter.
- Follows all designated SASH Coordinator duties outlined in the SASH Operations Manual; including coordinates and communicates with Discharge staff at off-site facility,
- updates SASH team on participant's transition status and confirm services to be available upon discharge,
- makes in-person visit to participant within 24-48 hours of return home to identify needs and review discharge instructions,
- coordinates and plans for any needed and unmet services with SASH team; and
- Updates participant's SASH health record and Healthy Living Plan (HLP).
- Develops the Community Healthy Living Plan (CHLP), which informs on-site, monthly educational events in collaboration with the SASH team and revises the CHLP Action Plan every 6 months based on ongoing assessment of the needs and opportunities of the participant community.
- Identifies how best to offer evidence-based, health and wellness educational services/programs for SASH participants (e.g., bring existing community-based programs on-site, assist participants to participate in off-site programs, design new programs).
- Provides encouragement and "coaching" to help participants keep focused and motivated on proper self-management of their chronic conditions.
- As requested, or agreed to by participants, provides reminders and/or daily check-ins to promote self-management skills.
- Organizes and coordinates on-site educational presentations on health and well-being topics based on the collective needs of the SASH participant population as outlined in the CHLP.
- Arranges and schedules assessments of SASH participants to determine health and functional needs.
- Conducts person-centered interviews with all SASH participants to understand their interests, needs, ideas, concerns and opinions.
- Coordinates the development of Individual Healthy Living Plans (HLPs) in collaboration with the SASH participant.
- Coordinates and assists SASH participants with individual support needs and goals as identified in the HLP.
- Communicates regularly and deliberately with SASH participants one on one and in groups to ensure consistent follow-up and information sharing among the SASH Coordinator, Wellness Nurse, SASH team and SASH participant; and
- Acts as key contact person on site for information sharing regarding SASH participants among the SASH team, Wellness Nurse, discharge-planning staff from hospitals and rehab facilities, family members and volunteers.
- Participates in any and all SASH team meetings.
- Consistent, daily input into the database used to track participant information and progress notes appertaining to SASH participants.
