Interim HealthCare is seeking a compassionate and skilled Registered Nurse (RN) to join our team  This is a full time position Monday -Friday 7:30am-4pm in an Assisted Living/ Memory care facility.  Some on call hours would be included at additional pay.  There is a also a potential for this to become a permanent position.  This RN will play a crucial role in ensuring the well-being and health of our senior residents.
The ideal candidate will have experience in geriatric nursing and a passion for providing high-quality care to elderly individuals.  Come join our amazing team of healthcare professionals

Hourly pay rate:  $55-$65/hour + on call compensation TBD

Responsibilities:

  • Case Management for residents in an assisted living/ memory care facility including comprehensive health assessments, medication management, and overall health status monitoring.
  • Leading a team of caregivers (CNAs and PCAs)
  • Collaborate with the interdisciplinary team to develop and update individualized care plans for residents, addressing their unique healthcare needs.
  • Provide health education to residents and their families on various topics such as disease management, preventative care, and healthy lifestyle choices.
  • Coordinate and communicate with other healthcare professionals, including physicians, therapists, and support staff, to ensure comprehensive and holistic care for residents.
  • Respond to medical emergencies promptly and efficiently, following established protocols and providing immediate care as needed.
  • Maintain accurate and up-to-date medical records, documenting all assessments, interventions, and communications related to resident care.
  • Work closely with the Director of Nursing, nursing staff, and other team members to maintain a cohesive and supportive care environment.
  • Foster open and transparent communication with residents, their families, and staff to promote a positive and collaborative care environment.

Requirements:

  • RN degree from an accredited school of nursing.
  • Current and valid RN license in the state of Minnesota.
  • Previous experience in geriatric nursing.
  • Excellent verbal and written communication skills, with the ability to communicate effectively with residents, families, and colleagues.
  • A compassionate and empathetic approach to working with senior residents.
  • Strong critical thinking and problem-solving skills.
  • Ability to work collaboratively within an interdisciplinary team.
  • Willingness and flexibility to cover on call needs over the phone.
