JOB SUMMARY:

The Health Equity Quality Coordinator will be part of a clinical quality reporting team. This role offers a unique opportunity to make a meaningful impact by addressing health disparities and promoting equitable access to healthcare. The Health Equity Coordinator engages in strategic planning, data analysis, stakeholder collaboration, and policy advocacy to drive positive change. By working directly with diverse populations and fostering partnerships, they have the chance to create innovative initiatives that address the root causes of health inequities and create a more equitable healthcare system.

The Health Equity Quality Coordinator will also abstract clinical data & review charts in paper and electronic formats; create dashboards/tables/graphs to help illustrate performance and potential Quality Improvement gaps related to disparities in Health Care and Social Drivers of Health (SDOH).

The goal and duties of this position are to assist the Clinical Report Nursing Supervisor in assessing, designating, and leading activities that will promote health equity and collaborate on creating resources for patients requiring health-related social needs. In addition, this position will also assist the team in successfully supporting patient quality and organizational outcomes on all assigned projects, meet deadlines and submission requirements for CMS reporting and support the team effort to continually develop their skills and contribute to improving the quality and safety environment of the appropriate facilities.

DUTIES AND RESPONSIBILITIES:

Serves as a Health Equity subject matter expert on various health equity initiatives as well as other goals and objectives for Clinical Quality Reporting set by Clinical Nurse Reporting Supervisor. Coordinates with Clinical Nurse Reporting Supervisor to meet data collection requirements to support timely completion and validation of all public reporting projects for Inpatient and Outpatient Quality Core Measures, CMS, and Commercial Payers. Schedules and coordinates stakeholder meetings, including fellow team members as appropriate. Monitors, coordinates and communicates the strategic objectives of health equity across the organization to optimize performance/results. Works to identify opportunities for continuous improvement, standardization, and health disparity reduction. Identifies communications strategies for dissemination of equitable outcomes research and recommendations. Partners with diverse teams and leaders across the community on health equity programs and initiatives. Manages community outreach and engagement programs and activities, and assists in the development of content and methodologies. Aggregates and assists with the analysis of internal and external health equity data.  Helps build diverse partnerships and collaboration to advance the mission and work of the organization Build and collaborate on reports that will help the organization(s) monitor quality of care and report metrics as needed/assigned by Clinical Nurse Reporting Supervisor. Complete appropriate data extracts, reports, and submissions and ensure they are handled in a timely manner and in accordance with program specifications. Communicates with Clinical Nurse Reporting Supervisors to share quality outcomes and opportunities related to the Clinical Quality Reporting team's work that may help various departments of the hospital(s). Works with Point of Care, Cerner, IT and other teams to identify technical solutions that can make current and on-going work more efficient and effective. Maintains knowledge of the business content, analytical methods, diverse data sources, and IT systems necessary to achieve unit, division and department goals. Supports and strengthens organizational priorities such as data transparency, security and compliance across analytics projects. Helps with Clinical Data entry as required for different measures including but not limited to electronic clinic quality measures. Monitor EHR clinical dashboard(s) to ensure capture of data for quality measures for the hospital(s). Other duties and responsibilities as assigned.

Qualifications

Education/Certification: preferred but not required any of the following: Degree in relevant field or equivalent experience (nursing, public health, epidemiology, healthcare administration, social work, etc).

Experience : Preferred three years or more of experience in the health care industry including but not limited to public health, epidemiology, healthcare administration, social work, and or discharge planning. Quality reporting experience and previous experience with clinical processes and patient care a major plus but NOT required. Experience with analytical environments (, data warehouse, business intelligence, SAP, etc.) Knowledge of process improvement methods ( LEAN, Plan Do Study Act). Strong understanding of healthcare equity and disparities or willingness to learn. Previous experience working in a team centered environment would be largely beneficial to the candidate.

Equipment/Technology: Use of computers and Electronic Health Record software is required. Experience with spreadsheet and database applications like Excel, Business Objects, SQL. Experience with clinical operations, billing and reporting requirements also preferred, but not required.

Language/Communication: Medical record review/abstraction knowledge in multiple specialties. Willingness and ability to manage multiple details with great care and accuracy. Successful track record of achieving results through persuasion and collaboration. Successful track record managing multiple complex projects simultaneously. Must exercise discretion, sound judgment, and the highest professional ethics.

Ability to identify and report trends in data that could indicate a process/procedure change needs to take place. Use of problem solving and planning skills with the Clinical Nurse Reporting Supervisor, to improve the quality metrics for the hospital(s) as assigned. Dynamic presentation skills and ability to develop clear and presentable documentation required.

Mathematical Skills:  Ability to do basic math and understand percentages, graphs, statistics.

Mental Capabilities: Strong attention to detail, problem solving, and organizational skills. Able to define problems, collect data and deal with stressful situations in a tactful professional manner. Knowledge of how to apply analytical methods and data to health care problem spaces

Interpersonal: Experience in meeting facilitation, and/or community relations. Demonstrates evidence of a team player attitude, communication, education, and collaboration skills. Proven ability to assist the team with creative problem solving. Uses a customer service oriented approach with a focus on building relationships with people at all levels of the Organization(s).

PHYSICAL DEMANDS: This position requires the manual dexterity sufficient to operate phones, computers and other office equipment. The position requires the physical ability to kneel, bend and perform light lifting. This person must have the ability to write and speak clearly using the English language to convey information and be able to hear at normal speaking levels both in person and over the telephone. Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. This person must have the ability to travel as required to work with different departments of the hospital and Outpatient ASC’s.
