About the Role

We are seeking a skilled Home Health RN Care Coordinator to join our team at St. Luke's Health System. As a key member of our care team, you will play a vital role in delivering exceptional patient experiences and building a positive, supportive, and inclusive culture.

Key Responsibilities
  • Develop and manage a plan of care from admission to discharge for an assigned case load, working closely with physicians and multidisciplinary team members.
  • Provide individualized care for patients from children to elderly, ensuring their unique needs are met and exceeded.
  • Manage case-loads effectively, ensuring daily visit requirements are met and exceeding state and national averages.
  • Travel within the Treasure Valley area to provide in-home care, utilizing your expertise to deliver high-quality patient care.

Requirements
  • ASN or BSN Degree
  • 2 years of RN experience, with Home Health or Acute Care experience preferred
  • Current RN License in the State of Practice and current Basic Life Support (BLS) Certification
  • Valid Driver's License and proof of Auto Insurance
