Senior Social Worker, Primary Care

As a Senior Social Worker you would be a member of the Elkton Primacy Care team in providing support to the Nurse Practitioner and Registered Nurse conducting home visits. Assists patients and families to identify social, emotional, and environmental needs and to connect them with available resources/services through the provision of a full spectrum of discharge planning and social work services. Essential functions are fundamental, core functions common to all positions of medical social work, in providing case management services to patients in a medical social services program area including geriatric patients and/or patients with disabilities of a physical, emotional or psychiatric nature. To support an innovative and integrated model of healthcare.
Work Schedule:
- Monday - Friday: 8a - 4:30p

- Minimum of a Master’s degree in Social Work required.
- Two to four years of experience in care management required.
- Travel to patient homes and other clinical locations is required.
- Must have a valid driver’s license and a registered vehicle for this travel.
- Must be a licensed master social worker (LMSW) in the State of Maryland.
