Would you be a great Licensed Practical Nurse?

To thrive as a Licensed Practical Nurse (LPN) at Comfort Home Care, you'll need a diverse skill set to ensure top-notch home care for our clients. Your role will involve conducting initial and ongoing assessments to accurately identify each client's medical needs while gauging their level of functioning and communication abilities. You'll initiate and revise the plan of care, performing OASIS assessments at the right times to keep our services aligned with their needs. Coordination is key, so you'll inform physicians and team members about any changes in clients' conditions, ensuring comprehensive care.

Collaborative skills are crucial as you'll partner with patients, families, and other healthcare providers to facilitate effective planning and discharge processes. Additionally, you'll need to possess strong organizational abilities to prepare clinical and progress notes, submitting all required documentation punctually in line with agency policies. Your problem-solving and empathetic nature will shine as you support clients and their families on their health journeys

Knowledge and skills required for the position are:

  • LPN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
  • LPN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
  • LPN Initiates and performs appropriate nursing interventions.
  • LPN coordinates services informs the physician and other personnel of changes in the client's condition and needs.
  • LPN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • LPN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
