Role Overview:
We are seeking a Program Coordinator to oversee a team providing coordinated healthcare and social services to underserved populations in Queens.

Compensation: $64,000.00
Location: Queens, New York
Schedule: Monday - Friday, 9 am - 5 pm 

Responsibilities: 
•    Supervising a Case Manager/Consumer Advocate team
•    Promoting a housing first and harm reduction approach
•    Identifying and enrolling eligible clients
•    Conducting assessments and care coordination
•    Establishing partnerships with healthcare providers
•    Engaging hard-to-reach clients and populations
•    Serving as a point person with partnered providers
•    Maintaining timely reporting and documentation

Qualifications:
•    MSW or related degree preferred
•    3+ years experience with underserved groups
•    Case management expertise
•    Motivational interviewing skills
•    Collaborative nature and problem-solving skills
•    Excellent communication abilities
•    Valid NY/NJ driver’s license
