Position Summary

The Home Infusion Intake Coordinator is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Infusion Intake Coordinator coordinates admission/intake activities for new/returning infusion therapy patients. S/he provides patient information to other branch departments to serve the patient and expedite patient billing.


Position Specific Responsibilities

Investigates/validates type and level of insurance coverage to determine eligibility for infusion therapy. Obtains and manages all prior authorizations for ordered therapy.Initiates admission process for patient referrals by collecting demographic and insurance information for patients. Enters all information into CPR+ software system. Informs team of referral within designated time frame.Acts as primary contact for new and returning infusion patients.Maintains positive pace from time of referral onset through start of care.Documents all findings within CPR+ and discusses with patient/patient representative. Completes and explains required forms to initiate infusion therapy.Communicates regularly with referral sources to ensure medical necessity is met for prescribed therapy(s).Documents medical necessity and compiles the required documentation for patients’ record ensuring information is available for any audit purposes.Supports designated staff in scanning and assigning documents into patient record.Arranges/coordinates home nursing referrals in collaboration with nursing manager.Maintains a customer service philosophy to ensure all customer expectations are met or exceeded.Assists with coordinating daily patient report/rounds. Maintains log of patient rounds/on-call report annually.Supports nursing with initiating patient education/care coordination documents in absence of in-office nurse.Participates in QI activities to assure compliance with accrediting bodies. Monitors CPR+ report lists for activity reporting. Assists in preparation of management reporting tools which are monitored across the system.Assists with implementing operational processes to assure admission operational efficiency as well as timely entry of patient information.Participates with billing team management of “on-hold” list.Participates in outreach activities as able.Serves as a role model/mentor for other staff in area of customer service/intake/insurance benefits.Perform other duties as assigned.


Education and/or Experience

High school diploma or equivalent.Associate or Bachelor's preferred.One (1) or more years of experience in a healthcare office setting; home infusion and/or specialty experience preferred.


Skills

Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications.Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.
