Position Summary:
The social worker is responsible to utilize judgment and critical thinking skills to identify and address the psychosocial needs of the patient or resident through assessment and care planning; collaborate with the providers and inter-disciplinary team to define and facilitate an effective, timely, safe discharge plan; refer and/or collaborator with internal and community resources to improve patient outcomes or mitigate barriers to health and services; create a work environment that is goal, culturally and age specific; ensure patients have timely access to needed services while respecting the rights and wishes of the patient and family. Achieve exemplary performance on quality and patient satisfaction measures.
Duties/Responsibilities:
• Complete psycho/social assessment in a timely manner.
• Provide social services (age and culturally specific) to patients.
• Assess financial situation and act on patient’s behalf for community, state and national resources.
• Identify and resolve problems that impede discharge planning or the patient’s use of health care resources.
• Collaborate with care team to develop, revise as necessary throughout the patient’s stay, and implement a discharge plan to ensure a safe and reliable transition to the next level of care.
• Provide education to patients and caregivers identified services.
• Maintain a resource guide of area agencies including HME, Home Care, Home Infusion, Nursing Homes for patient choice and post discharge services.
• Maintain a resource guide of community resources, providers and partners.
• Maintain criteria to prioritize high risk patients.
• Assist patients with advance directives.
• Advocate for patient rights and protection.
• Assists as mediator with patient/family/facility conflicts or ethical dilemma.
• Serve as authority in programs relating to community resources.
• Timely process, report and coordinate investigations of abuse, neglect, guardianship and other cases to the appropriate advocacy, criminal, regulatory and/or authority agency.
• Documents social case work and discharge plan. Timely and according to established SCH policies and procedures.
• Communicate and collaborate with providers, social workers and care team to ensure safe and effective care transition.
• Complies with licensure laws, NASW Codes and Ethics.
• Actively participate in SCH Quality and Performance Improvement activities.
Education:
Bachelor’s Degree required. Education as a Generalist and be able to apply skills to all age groups. Master’s Degree in Social Work preferred.
Experience:
Two (2) years clinical experience, preferably in acute care social work, care management and discharge planning.
Licensure/Certification:
Must become licensed in Kentucky by the State Board of Examiners of Social Work within 18 months from date of employment.
