Responsibilities:

  • In charge of providing direct patient care to patients, including assessing and monitoring patients' conditions, administering medications, and performing treatments at least once a week
  • Collaborate with healthcare team members to develop and implement individualized care plans
  • Coordinate and facilitate communication between patients, families, and healthcare providers
  • Adhere to HIPAA regulations to protect patient privacy and confidentiality
Qualifications:

  • Minimum of 1 year of experience as a Registered Nurse in a hospice
  • Active Registered Nurse (RN) license
  • Must be able to drive, have a reliable vehicle, and have valid insurance
Skills:

  • Excellent patient service skills, with the ability to provide compassionate and empathetic care
  • Energetic and positive attitude
  • Strong critical thinking and problem-solving abilities
  • Effective communication skills to collaborate with interdisciplinary healthcare teams
