We are seeking a compassionate and skilled RN Hospice Case Manager to join our team at Phoenix Home Care and Hospice. As a RN Hospice Case Manager, you will provide high-quality nursing care to patients and their families, working collaboratively with the interdisciplinary team to develop and implement a patient-centered plan of care.
Responsibilities- Provide education and guidance to patients and their families regarding the hospice care process and plan of care.
- Consult with the attending physician and Hospice Medical Director to ensure seamless care coordination.
- Coordinate hospice care services, working closely with patients, families, and other team members to ensure needs are met.
- Conduct comprehensive assessments and evaluations to determine the best course of care.
- Kansas RN License
- Hospice experience preferred but not required
- Valid driver's license and auto liability insurance
