Primary Purpose:

Coordinate Pre-K through grade 12+ transition planning and services from ECI or home to Pre-K, from grade level to grade level, and from public school to secondary education or competitive employment for students with disabilities.

Qualifications:

Master’s degree preferred in special education or school administration Valid Texas Teacher Certificate or licensure in a related field Thorough understanding of IDEA and transition planning for students with disabilities Knowledge of community-based programs, school-to-work programs, and agency linkages Ability to provide leadership for teachers in transition-related issues Knowledge of secondary and early childhood transition (arena evaluations, a summary of performance, etc.) Knowledge of research, trends, and best practices in special education Knowledge of state and federal laws, rules and regulations, and policies applicable to special education Ability to maintain a positive relationship with school personnel, parents, and community Ability to communicate effectively orally and in writing Knowledge of special education assessment and eligibility practices

Major Responsibilities and Duties:

Coordinate effective and legally compliant transition planning services for students receiving special education services. Collaborate with campus special education teachers and staff to ensure high quality and legally compliant transition planning to promote competitive employment opportunities for students with disabilities. Coordinate the planning and implementation of Transition Focus Groups Coordinate with VAC and RISE coordinating teachers to facilitate meetings for transition planning and agency support at the secondary schools Collaborate with outside agencies (Health and Human Services Commission; Department of Aging and Disability Services; Department of Assistive and Rehabilitative Services; Department of State Health Services; and Department of Family and Protective Services as appropriate to ensure effective post-secondary planning Coordinate services in early childhood transition from ECI or home to early childhood programs Provide transition training for all middle and high school special education teachers Assist in the planning, coordinating, and implementing of NISD programs in transition early childhood through 12+. Assist in the coordination of transition planning for students with disabilities transitioning into Kindergarten Coordinate procedures for identifying the needs of childcare providers in transition plans Assist with ready schools plans and graduation plans for students with disabilities Attends transition alignment meetings Coordinate with RISE and maintain job site lists Coordinate with VAC coordinating teachers to streamline services between high school campuses Support all forms of communication with staff, students, and parents Monitor, evaluate, edit, transition plans, IFSP plans, and service plans Compile data from surveys creating and prepare reports Create and maintain a database of students with agency support Compile l data for SPP indicators 13. Coordinate a continuous in-service program designed to improve the professional staff Research, plan, and coordinate in-service training programs for administrators, parents, general education teachers, special education teachers, and itinerant staff in understanding special education Coordinate scheduling of monthly PLCs, focus groups, and parent training Interpret and disseminate information regarding state/federal rules and regulations Inform NISD staff of student progress, teacher and other personnel effectiveness, and program development Develop and provide district training for special education Other duties as assigned

SHARS/ MAC / RMTS:
Northwest ISD participates in the School Health and Related Services (SHARS) and MAC programs which allows our district to request Medicaid reimbursement for certain health-related services. The Admission, Review, and Dismissal (ARD) committee determines SHARS services.

As a special education staff member, you will be required to train and bill for SHARS reimbursements for students on your caseload. Trainings will be provided for SHARS, for RMTS (Random Moment in Time Studies), where staff are chosen randomly to answer a few questions, and for MAC (Diagnosticians and non-Personal Care Providers only). Training will be provided at the beginning of each school year as well as on an “as needed” basis for new hires.

Working Conditions:

Maintain emotional control under stress. Regular district-wide travel to multiple locations as assigned; frequent standing, stooping, bending, kneeling, pushing, and pulling, moving of small stacks of textbooks, media equipment, desks, and other classroom or adaptive equipment. District-wide travel and occasional statewide travel may be required. May be required to lift and transfer students to and from wheelchairs or assist with positioning students with physical disabilities. Position requires frequent bending, stooping, twisting, pulling, overhead reaching, sitting, and keyboarding. Position requires the ability to lift 25 pounds to waist high.
