HeartShare is looking for a Social Worker

The Social Worker is responsible for compliance with all federal, state and Office of Mental Health (OMH) requirements to ensure the continuum of services necessary for each child residing in the Children's Community Residence. The social worker provides case management and social services to residents and families. The Social Worker coordinates and delivers services to clients and families and coordinates the intake and discharge process.

Responsibilities:

Responsible for intake and discharge coordination; maintain waitlist. Develop relationships with community organizations and resources to improve service access for children in the program. Coordinate treatment services to ensure continuity of care. Facilitate psychoeducation to the residents and staff. Maintain frequent contact with the children’s families and guardians; arrange visits with families and/or siblings; provide services to the child and family and prepare the family for reunification. Provide counseling and crisis intervention to all residents. Timely follow-up on incidents. Collaborate with the Benefits Entitlement Specialist to ensure timely and accurate processing of Medicaid payments and SSI. Ensure timely PNA payments to youths. Coordinate and facilitate interdisciplinary team meetings, family meetings, attend relevant meetings and trainings; including but not limited to: incident review, school, service providers, house meetings, and other meetings necessary. Ensure all required documentation for a complete case record including intake information and consents, admission note, service plans, monthly progress notes, all assessments, service notes, aftercare follow up (30/60/90), entitlement information and other documentation as required. Follow up to ensure youths attend appointments and have follow up conversations. Work flexible hours as residents’ needs require. Must be able to respond to program emergencies within or outside of office hours. Timely updates to CAIRS database – admissions and discharges; completion of monthly program roster, Compile and submit HRA 2010e packets for supportive housing applications and assist with applications for other housing options. Collaborate with the nurse to address medical-related concerns. Collaborate with recreation specialists to ensure activities support the goals of the youths. Conduct relevant field visits to support the youths as required, such as home, hospital, escort to appointments etc. Perform other duties necessary to support the program and youths.

Qualifications

Licensed or Master’s level mental health professional. MSW preferred. NYS driver’s license preferred. Must have at least 2 years professional childcare or direct clinical support services experience. Must obtain First Aid Certification. Must be available to work flexible hours. Experience as a clinician for clients with highly specialized needs and multiple psychosocial risk factors preferred. Must support, nurture and counsel residents in a structured environment. Delivering services through a trauma-informed lens using evidence-based models such as Motivational Interviewing, Intensive Permanence Services etc. Must have written and verbal communication skills sufficient to complete required documents and reports, and to participate in and facilitate meetings. Experience in working with diverse communities, and knowledge of multicultural issues including race, class, ethnicity, sexual orientation, gender expression, etc. Operating skills – keeping on point, getting organized, managing work processes. Leadership skills Personal & interpersonal skills – relating skills, managing diverse relationships, inspiring others, being open and receptive. Must comply with all agency policies and procedures. Familiar with OMH Part 594 Rules and Regulations for the operation of Children’s Community Residences.
