Lifescape is the largest non-profit organization serving older adults in northwestern Illinois. We serve thousands of older adults annually and have proudly established mental health services targeting older adults. This position plays an integral part in our programming that supports our strategy of providing mental health services for older adults. We seek a highly self-motivated, mental health professional, with an interest in program and grant execution.

This position is funded by the Winnebago County Community Mental Health Board. The position is responsible for executing the Healthy IDEAS (Identifying Depression & Empowering Activities for Seniors) evidence-based program and AMP (the Aging Mastery Program) in Winnebago County.

Essential Duties and Responsibilities include the following. Related duties may be assigned.


  • Outreach and Identification of older adults living with depression
  • Education to individuals, families, and professionals about mental health in older adults, including identification of signs and symptoms, what help is available, and who to contact for help
  • Screening of older adult for depression utilizing depression scales
  • Provision of case management to older adults across the community, including within client homes to address protective factors
  • Navigate and connect at risk older adults with licensed mental health treatment
  • Monitor and Achieve outcomes as established by grant funding and program director
  • Develop and/or prepare program reports
  • Participate in grant writing and program development opportunities
  • Represent Lifescape at the Learning Network Collaborative
  • Develop collaborative agreements and referring relationships with mental healthcare providers
  • Participate in department leadership meetings
  • Ensure program quality, compliance, and fidelity to evidence-based research protocols
  • Stay informed on the Winnebago County Community Mental Health Board (Grantor) meetings and grant requirements.
  • Provide guidance, scheduling, and supervision of the mental health Support Group.
  • Participate in operations planning and strategy development for future growth
  • Supervise and evaluate employees in a fair and consistent manner and in compliance with agency policies, funding, state and federal requirements.
  • Develop, conduct, and maintain in-house training for all appropriate personnel to ensure agency compliance with all funding, state and federal guidelines in service provision
  • All other duties as assigned

Minimum Qualifications

  • Bachelor's degree in social work, psychology, public health, or related field, or equivalent experience, including experience working with older adults
  • Supervisory experience
  • Must be 21 years of age
  • Must have valid driver's license, clean driving record and own transportation with current insurance coverage. Not have been convicted within 3 years of the date of application of any of the following offenses involving the operation of a motor vehicle: Driving Under the Influence of alcohol and/or drugs (DUI), Reckless Driving, Leaving the Scene of an accident involving death or personal injury, Drag Racing, Manslaughter, Reckless Homicide, Reckless Conduct
  • Must be able to communicate clearly and professionally with individuals at all levels, both orally and in writing
  • Must be able to read, understand and coordinate complicated instructions and guidelines to meet federal and state requirements as governing services to the elderly
  • Must be able to travel to and enter various locations to participate in meetings as assigned or required
  • Must be able to mediate concerns and/or conflicts among clients, staff and volunteers
  • Must be able to work independently, and function under varying circumstances with flexibility
  • Must have physical mobility to perform job-related duties outside the office environment
  • Must be able to lift a minimum of 40 pounds in setting up displays and equipment for events and activities
  • Must be able to perform essential functions of position
  • Must be able to work nights and weekends, as needed

Education and Experience:

  • Bachelor's degree in social work, psychology, public health, or related field, master's degree in related field preferred
  • 3 years' experience working with older adults in health or human service capacity
  • 3 years mental health services experience or related field
  • 3 years supervisory experience
