Job Summary

Rosecrance Inc. is seeking a highly skilled and compassionate Case Manager - Mental Health Specialist to join our team. As a key member of our Assertive Community Treatment (ACT) Team, you will provide critical support to individuals and families affected by mental health challenges.

Key Responsibilities
  • Case Management: Provide individualized case management services to clients, including assessment, goal setting, and coordination of care.
  • Crisis Intervention: Respond to crisis situations and provide stabilization and support to clients and their families.
  • Documentation: Maintain accurate and timely documentation of client interactions, progress, and outcomes.
  • Collaboration: Work closely with multidisciplinary teams, including healthcare providers, social workers, and other stakeholders to ensure comprehensive care.
  • Education and Training: Develop and deliver educational programs and workshops to clients, families, and community groups on mental health topics.
Requirements
  • Education: Bachelor's degree in Psychology, Human Development, or a related field, or equivalent experience in mental health.
  • Experience: Minimum of two years' experience in mental health case management or a related field.
  • Skills: Excellent communication, organizational, and problem-solving skills; ability to work independently and as part of a team.
