Dir Case Management

Position Information:
Responsible for planning monitoring and is accountable for the management and delivery of Case Management services at all system locations. Provides daily operational oversight management and leadership for staff of the department whose activities are associated with the continuum of care clinical cost reduction utilization management care transitions population management clinical social work quality performance improvement reimbursement and discharge planning. Ensures efficient and professional services designed to meet the needs of the patients personnel medical staff hospital and health system strategic objectives. Serves as a clinical resource to the Case Management staff Revenue Cycle and other hospital staff regarding care coordination issues social work financial counseling utilization management and substance abuse. Works closely with nursing management physicians clinical managers and other management staff to ensure that services are being provided in a timely manner ensuring coordination between departments and improved resource utilization. Ensures the hospitals are meeting their license requirement of serving patients with acute hospital needs and developing plans to move patients to other settings that do not require acute hospital resources.
Experience Requirements:
5 Years Related Position Experience Required - And 2 Years Leadership Required

Bachelor's Degree Nursing Required - And Master's Degree Healthcare Required - Or Master's Degree Social Work Required
Licensure/Certification Requirements:
Registered Nurse Required - Or Licensed Clinical Social Worker Required - And Certified Case Manager Or Accredited Case Manager Required - Within 36 Months of Hire
Hours of Work:
Monday - Friday, 8:00am - 4:30pm
