Job Requirements:
- · Comprehensive knowledge and skill in current nursing practice.
- · Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.
- · Knowledge of the OASIS process.
- · Knowledge of the community resources.
- · Ability to make meaningful observations, evaluations and reports.
- · Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.
- · Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.
- · Valid driver’s license.
· Minimum of 1 year Home Care Experience as a Case Manager
