What you’ll be doing as a RN: Complete nursing assessment, monitoring, evaluation, interventions, and treatment to care for acute care patient populations in the home. Collaborate with the patient, family, interdisciplinary team, therapists, specialists, and use community resources to provide effective care coordination; evaluate social determinants of health and incorporate into the patient's individualized care plan and care management. Assist with biometric monitoring technology and respond to biometric vital sign data. Provide individualized patient education on disease and symptom management, medication management, treatments, procedures, and therapies. Prepare for and coordinate discharge from hospital at home with patient and family to ensure seamless transition to patient's PCP, home care services, and specialists, with community resources as appropriate. Competence in procedural skills such as: phlebotomy, vascular access device management, hydration therapy, medication administration, wound management, catheter care, specimen collection, stoma management, and compression therapy. Participate in quality and safety investigations and root cause analyses; take appropriate action to ensure patient safety as a result of any investigation.
