Responsibilities:
Coordinate intakes and discharges in adherence with policies and procedures.
Monitor utilization of service hours to avoid over or under utilization.
Provide direct care and support to residents as needed.
Oversee day-to-day operations of the group home program.
Collaborate with healthcare professionals, families, and external service providers.
This position is on-call and will serve as a part of an on-call rotation.
Advocate for the rights and well-being of residents.

Location, Scheduled & Pay:
We have various locations across the Twin Cities metro with various schedules.
This position is typically Monday-Friday, standard business hours.
Starting salary of $50k annually and goes up depending on education and experience with a $750 hiring bonus
What qualifications are required?
High School Diploma or GED equivalent preferred.
Bachelors degree in a human service related field preferred.
Must have two years of professional experience working with individuals with special needs. One year of supervisory and program coordination experience preferred. Excellent attention to detail and organizational skills.
Valid drivers license is required.
