Job Title: Patient Care Coordinator / Patient Services Representative

Company: Home Health Care, Inc.

Job Type: Full Time (Monday to Friday, 8:00am to 5:00pm)

Compensation: $16 - $17.50 per hour

Location: Golden Valley, MN

Home Health Care, Inc. has the perfect opportunity for you to continue your career growth Our team of Patient Care Coordinators are instrumental in providing a seamless patient experience and making sure all documentation and information is collected and organized appropriately. Successful candidates will have a passion for providing a high level of customer service to home care patients. Additionally, selected candidates will have had prior customer service experience in a fast-paced environment.

Patient Care Coordinator Responsibilities:
•Send and track required face-to-face documentation for Physicians to sign and return
•Collect information for currently hospitalized clients: H&P (History & Physical), ROC (Resumption of Care), MD orders, etc.
•Conduct pre-admit calls to verify client information, as well as five day and discharge follow-up calls
•Track hospitalized clients and notify Community Liaisons of discharges, in order to resume providing care

Patient Care Coordinator Qualifications:
•High school graduate; college degree preferred
•Knowledge of medical terminology is strongly preferred
•Minimum of one (1) year of experience in an office or health care setting preferred
•Strong verbal, written, and telephonic communication skills
•Demonstrated ability to type at least 50 words per minute
