The Case Manager will coordinate and provide support and services to Participants through the process of assessing, planning, implementing, coordinating, monitoring and evaluating options and services required to meet the Participant’s service plan needs and goals around mental health, physical and general wellbeing. Through mentoring, advocacy, communication and resource navigation, the Case Manager will promote equity and cost-effective reintegration processes that will help find services needed and monitor progress through the completion of a service plan and progress report. Services include but are not limited to: connecting Participants with additional critical resources, such as mental health care, medical care, legal se8rvices, victim’s rights and/or compensation, interpretation services, immigration services, workforce development and financial education. Emotional support navigating the criminal justice system, including police departments, legal services, and the courts. Services take place at locations designated as safe spaces in the community and other locations as needed DUTIES AND RESPONSIBILITIES Adheres to the mission of Central States SER Coordinating resources by established CBOs partners and internal programs offered by SER and with direction provided by the convener as instructed by Illinois Criminal Justice Information Authority. Conducting intake in a professional manner that reflects emotional and cultural awareness Collecting and recording accurate information using Customer Relationship Management (CRM) software such as Munission and Microsoft Office. Compile and maintain accurate records in our database to monitor the program's progress, including case notes on well-being check-ins, acclimation, personal action plans, milestone achievements, attendance records, class progress, and employment verification. Complying with all Illinois Department of Corrections, IDOC or Illinois Parole regulations, including those aimed at protecting personal information Protect and maintain individuals’ privacy and confidentiality Provide support and consult with Reintegration Coordinator to meet participant’s needs The Case Manager possesses the education. skills and experience required to render appropriate services based on sound practice principles. Administer pre- and post-release needs assessments to determine services and programmatic needs of returning citizens. Identify and refer participants to community resources, including housing, wrap-around services for mental health and substance use treatment, public benefits, healthcare, etc. Review reintegration plans with the participant to develop realistic goals and personal action plans to ensure successful reintegration and address immediate and long-term needs. Serve as an advocate for the participants to community organizations (i.e. transitional living facilities, parole officer) and family to ensure networks of support are communicating in a productive manner. Communicate and provide the Reentry Coordinator with progress reports, challenges and new goals of each participant in their monthly Case Manager’s meetings. Build partnerships, utilize existing partnerships, and liaise with other organizations and employers working on reentry efforts. Communicate with participants to do wellness check-ins and progress updates on personal action plans. Arrange a regular schedule for communication and follow-ups to encourage the building of relationships with Participants and ensure the completion of service plans Utilize motivational interviewing practices to engage and retain Participants through the completion of their service plans Compile and maintain accurate records in our database to monitor the program's progress, including case notes on well-being check-ins, acclimation, personal action plans, milestone achievements, attendance records, class progress, and employment verification. Maintains current knowledge in the field through designated training, coursework, seminars and workshops as determined by the Reintegration Coordinator Contributes to data reporting requests in collaboration with the Reintegration Coordinator. Participate in ILRCC program meetings to coordinate information on the Participant’s release/parole dates and needs and document the progress and well-being of the Participant. Ensures that program design and implementation processes are followed. Makes referrals to collaborative partners for services that cannot be provided in-house. Performs other related duties as assigned

JOB REQUIREMENTS: A High School Diploma or GED is required, but College-level education is preferred Required experience in dealing with justice-involved individuals, those incarcerated, judicial processed, and hard-to-serve populations. Proficiency in utilizing Windows Operating system and performing data entry; previous experience in CRM software is an additional benefit Advanced proficiency in English is required, bilingual Spanish proficiency is preferred Proficiency in MS Word, Excel, and Outlook Express, as well as data entry Ability to comply with regulations and communicate official information in an appropriate manner Advanced ability to coordinate efforts, facilitate integration, and maintain records Exceptional organizational skills and sound judgment Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds
