**Position Purpose**

This role provides supervision and guidance to TPAN's medical case management team.

**Essential Functions**
- Oversee, manage, delegate and direct work assignments of the case managers.
- Provide case management services to clients with a caseload of up to 10 clients.
- Hire, train, and supervise staff. Meet with case managers for supervision on a weekly basis and as a team on a weekly basis.
- Ensure that case managers are completing care plans, assessments, face to face meetings, measuring viral suppression, attending case conferences, monthly contacts, and clients are attending medical appointments, all in compliance with our funders.
- Review case notes and ensure all are compliant and consistent with the goals document in client’s care plans.
- Ensure that all case managers complete AFC required trainings per calendar year.
- Ensure that Medical Case Managers meet all scopes and objectives as stipulated by grant contracts.
- Oversee and monitor the timely completion of program reports on a quarterly basis with AFC
- In conjunction with the Data and Technology Director, conduct chart reviews on a quarterly basis, and make sure data is entered into Provide and Athena Health on a regular basis.
- Coordinate and oversee TPAN’s Client Advisory Board.
- Work in conjunction with TPAN’s 340B committee to ensure the team is enrolling all qualified clients.
- Develop and maintain effective working relationships with other service providers, network providers and outside programs and agencies.
- Attend any required AIDS Foundation of Chicago management meetings.
- Attend staff meetings.
- Perform other duties as assigned.

**Qualifications**
- Bachelor’s degree preferred.
- Have a minimum of 2 years (required) or 4 years (preferred) as a Medical Case Manager or a job with similar duties.
- Demonstrated cultural competence in working with people of color and the LGBTQ community as well as experience and knowledge working with those living with and affected by HIV/AIDS.
- Strong conflict management skills.
- Knowledge of HIV/AIDS, mental health, and substance abuse evidence-based practices
- Strong critical thinking and reasoning skills.
- Knowledge of health insurance, public benefits, and resources available in the Chicagoland area.
- Strong interpersonal, organizational, and written and verbal communication skills.
- Demonstrated excellent oral and written communication skills.
- Proven ability to collaborate successfully across cultural and social communities.

**Salary**:
Starting at $53,000/annually (Depending on Experience)
