Registered Nurse (RN)

    • Performs in an independent capacity.
    • Provides direct and indirect patient care and coordinates patient care.
    • Manages the overall treatment of assigned patients.
    • Maintains communication with patient, family, and physician
Essential Functions:
  • The RN complies with all organizational policies regarding ethical business practices and maintains regulatory requirements as governed by their licensing.
  • The RN ensures compliance with policies and procedures regarding clinical operations, fire, safety, and infection control.
  • The RN utilizes the nursing process to evaluation patients and plan, coordinate, and implement the plan of care.
  • The RN regularly reevaluates the patient's needs, initiates any revision of the plan of care as needed to assure optimum patient care.
  • The RN initiates appropriate preventive and rehabilitative nursing procedures.
  • The RN carries out physician orders.
  • The RN skillfully performs hands-on techniques and treatments necessary for safe patient care in accordance with professional standards.
  • The RN administers medications accurately and documents it as per physician's orders.
  • The RN inspects the patient's home environment for any possible dangers or hazards.
  • The RN prepares and maintains professional, accurate and complete clinical records.
  • The RN responds in a safe and appropriate fashion to patient's emergencies.
  • The RN assists and instructs patients and their family in self-care techniques.
  • The RN acts as a resource for patients and their families.
  • The RN informs the physician and other personnel of changes in the patient's condition and needs, counsels the family and the patient in meeting nursing and related needs.
  • The RN appropriately delegates tasks to appropriate health care staff members.
  • The RN effectively and consistently communicates administrative directives to personnel and encourages interactive departmental meetings and discussions.
  • The RN supervises the home health aide and the LVN's.
  • The RN evaluates the home health aide competency, performs in-services for the home health aide, and completes annual performance evaluation of the home health aide.
  • The RN monitors patient care by the therapists and social workers.
  • The RN maintains a working knowledge of current Medicare, Medicaid and third party payer OASIS requirements for home care reimbursement.
  • The RN participates in THCS information management, performance improvement, quarterly clinical record review, and QAPI programs.
  • The RN participates in in-service programs.
  • The RN participates in case conferences and all other mandatory meetings.
  • The RN participates in continuing education programs.
  • The RN conducts themselves in a professional manner, both verbally and nonverbal.
  • The RN promotes a positive attitude at all times.
  • The RN performs other duties as assigned or as required.
Education:
  • Graduate of an accredited school of nursing
  • Fully licensed in the State of Texas
  • Able to maintain and improve management/supervisory skills
Training Requirements:
A minimum of two years nursing experience in home health services preferred.
  • At least one year supervisory and administrative experience.
  • Basic knowledge in computer use.
  • Must be CPR certified and have current TB skin test.
  • Preferred certification in performing home IV therapy procedures.
  • Training and experience in wound care, drawing blood, and technical catheter procedures.
Knowledge Requirements:
  • General nursing procedures and techniques involved in the care of patients, disease processes and possible results, medical terminology, HHA processes and equipment.
  • Able to make independent decisions when circumstances warrant such action.
  • Must be able to read, write, speak and understand the English language.
  • Understanding of HHA basic functions and goals.
  • Basic understanding and knowledge of Medicare, Medicaid policy and private insurance.
  • Knowledge and mastery of Point Of Care device.
Professional Requirements:
  • Completes the annual educational requirements as required to maintain nursing license.
  • Maintains and ensures HIPAA compliance.
  • Responsible and accountable to the Supervising Nurse, Assistant Supervising Nurse, the DON, the Administrator and the CEO about time availability and performance.
  • Maintains good rapport and cooperative working relationships with patients, physicians, referrals sources, regulatory agencies and staff.
  • Adheres to company dress code.
  • Attends annual review and departmental in-service training.
