About the JobGeneral Summary of PositionThe Therapy Care Manager plays a vital role in guiding the field training experience for newly hired clinicians mentoring them throughout their first year to promote engagement retention and the development of clinical expertise. This role ensures high-quality care delivery by providing clinical support training guidance and fostering sound decision-making among field staff. Key responsibilities include performing patient care and case management as needed in collaboration with the Director of Therapy Services and the Clinical Operations Manager. Additional duties involve supporting quality care initiatives case conferencing staff coordination and managing clinical workflows.In partnership with the education quality and operations teams the Therapy Care Manager assists the Professional Training Specialist in developing the clinical orientation programs for new hires facilitates their integration into the team and serves as a field trainer to promote best practices. This role actively contributes to quality assurance through joint visits chart audits participation in mock surveys and supporting fall occurrence resolution.Serving as a knowledgeable and collaborative resource this role provides clinical support to field staff under the supervision of the Director of Therapy Services AVP of Operations and in collaboration with Professional Training Specialist and Clinical Operations Manager within assigned teams.Primary Duties and ResponsibilitiesContributes to the achievement of established goals and objectives established by the Medstar management team. Adheres to department policies procedures quality standards and safety standards. Completes annual Code of Conduct Compliance and other mandatory training. Complies with governmental and accreditation regulations. Partners with Quality and Clinical Operations Manager to assist with patient complaints and fall occurrence resolution. Communicates with patients caregivers and interdisciplinary staff to resolve actual or potential issues with the patient. Conducts chart reviews and assures that admissions recerts and discharges in the patient documentation are accurate and that proper utilization and clinical needs are being achieved. Leads preceptor responsible for training and providing feedback to new associates doing peer-to-peer reviews and being available as a clinical resource for field staff. Maintains a caseload of patients and meets the expectation of care described in the corresponding clinical job description. As deemed necessary the role of Care Manager also performs admissions (as appropriate) revisits recerts discharges and joint visits to ensure quality patient care delivery and excellence in patient experience. Maintains effective working relationships with other departments. Participates in multidisciplinary quality and service improvement teams meetings and on committees representing the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. Participates in Mock surveys as directed. Performs Daily Workflow duties as designated per team. Serves as an educator in the discipline providing feedback and suggestions to improve workflow best practices and standard procedures. Participates in meetings and on committees and represents the department and organization in community outreach efforts. Participates in multidisciplinary quality and service improvement teams. Provides Clinical Operations Manager with input regarding team members' work performance. Provides oversight of staffing and scheduling concerns. Provides weekly case conferences to orienteers during IPR and continues on a bi-weekly basis after the IPR period. Responsible for clinical outcomes and utilization management. Serves as Clinical resource to patient caregiver physician. Minimal QualificationsEducation Bachelor's degree in Physical Therapy required or Master's degree in Physical Therapy (MPT) required or Doctoral degree in Physical Therapy (DPT) required Experience 1-2 years Experience in a certified home care or home hospice program required Licenses and Certifications PT - Physical Therapist License to practice Physical Therapy in all jurisdictions in which the employee provides services to patients the District of Columbia Maryland or Virginia or any combination based on work location(s) Upon Hire required and DL - Valid State Driver's License in good standing and a dependable vehicle for transportation Upon Hire required and CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and Additional unit/specialty certifications may vary by department or business unit. Knowledge Skills and Abilities Ability to case manage. Organization skills. Excellent verbal and written communication skills. Basic computer skills preferred. Knowledge of home equipment home care reimbursement and regulations governing PTA's LPN and HHA's. This position has a hiring range ofUSD $89,065.00 - USD $162,801.00 /Yr.
