PURPOSE OF THE JOB

Coordinates resident care plans to promote wellness and meet health needs of the residents and provides education and competency evaluation of licensed and unlicensed staff providing direct care and medication assistance for residents.

o Ensures that residents receive the best care possible by encouraging open communication with the management team, direct care personnel, residents and families.

o Follows all community policies and procedures related to investigating, reporting and preventing abuse of vulnerable adults.

KEY POSITION SPECIFIC ROLES & RESPONSIBILTIES

  • Prior to resident admission, completes evaluation to identify resident care needs
  • Develops resident service plans that identify resident care and needs
  • Collaborates with community nurse on communication of changes in resident health needs and medical

follow up, identification of allergies, medication, and care interventions related to ongoing medical

conditions and health teaching needed

  • Coordinates reviews of resident service plans with resident (Care Conference) and care staff and works

with community nurse on updates to plans as needed to meet identified needs.

  • Identifies and coordinates services with other health care providers (Ex Home Health, Pharmacist)
  • Supports community nurse on promoting resident independence including medication selfadministration

assessments

  • Sets up medication and medication plans as needed in collaboration with community nurse
  • Documents evaluations and care in resident care plan
  • Participates in staff orientation and training activities
  • Participates in staff competency evaluation, documents and reports areas of concern or lack of

competency to the Director of Wellness.

  • Track ancillary services report to BOM by monthly deadline for resident invoices
  • Assist with scheduling as needed
  • Participates in committees as requested by the Executive Director and Director of Wellness (Quality,

Safety, etc)

  • Participates in community and other public education regarding care and services provided at

community.

  • Supporting community nurse in maintains resident care plans in accordance with professional and

regulatory standards

  • Assist on the floor as needed
  • Plans and implements reviews of Pharmacy records and provides oversight on medications being

administered.

  • Develops and implements health and wellness educational opportunities for residents/staff.
  • Provides appropriate orientation, training, direction and mentoring for all care staff, including Quality

Standards and Core Values.

  • Assist in other responsibilities as needed
