Supported Living Coordinator

JOB SUMMARY
The Supported Living Coordinator works with clients, families, support staff, Case Managers, community members, and agencies to assure that in every aspect the individuals served receive the highest quality of services. The SLC will participate as an active member of the Leadership Team. This is a safety-sensitive position.
The Supported Living Coordinator provides supervision, training, and support to Direct Service Professionals. The SLC implements services specified in client plans and supervises staff working with designated clients. The SLC is the key role model for running successful teams and providing the support for other teams. The Supported Living Coordinator is responsible for communicating programmatic and organizational information to all clients, families, and staff on their team. The SLC will make provisions for service delivery as needed to ensure that the individuals served by Life Styles receive the services that the organization has agreed to provide.
JOB RESPONSIBILITIES
1. Staff Training & Supervision:
- Provide orientation, training, and supervision for direct service staff or assign a designee.
- Oversee the implementation of the Person-Centered Support Plan (PCSP).
- Develop and update Individual Program Plans (IPP).
- Attend or assign staff to Quarterly Review Meetings.
- Make updates to IPPs as needed.
- Ensure the services outlined in the Plan of Care are delivered.
2. Quality Monitoring & Compliance:
- Conduct two client check-ins per month (at least one in person).
- Review documentation and timesheets every two weeks for accuracy, compliance, and concerns.
- Communicate with family members at least once every three months, or more frequently when possible.
- Maintain client records in accordance with Medicaid, DDS, and agency requirements.
3. Financial Oversight:
- Follow the Client Finance Policy for managing client finances.
- Attend required staff training and team meetings.
- Collaborate with the team to ensure clients receive high-quality services.
JOB REQUIREMENTS
- Bachelor's degree (BA or BS) or 5 years' experience with developmental disability support
- Valid driver's license and an insured vehicle
- Pre-employment drug screen
Job Type: Full-time
Pay: $45, $50,000.00 per year
