About the Role

We're expanding our award-winning outpatient model into homes across Jacksonville, Ponte Vedra, Nocatee, and Northern St. Johns County.

This is not home health. It's real outpatient physical therapy delivered in the home for patients who expect the same level of care they'd receive in our clinic.

There are no OASIS forms, no PTAs, and no endless admin work—just patient-focused care and the freedom to treat the way you were trained.

Pay:

  • Equivalent to $80,000 to $90,000+ per year for 25 or more visits each week
