We are partnered with several expanding home care agencies, and one is seeking a friendly, organized, and professional Bilingual Home Care Intake Specialist based in Brooklyn This is a referral placement opportunity through our network.

Key Responsibilities

As a Bilingual Home Care Intake Specialist, you will be the first point of contact for potential clients and their families, ensuring a smooth onboarding process. Your responsibilities will include, but not be limited to:

  • Client Onboarding: Managing the intake process for new patients, ensuring all required documentation is completed and accurate.
  • Assessment Coordination: Scheduling and coordinating in-home assessments with clients and care staff.
  • Communication: Acting as a liaison between clients, caregivers, and other team members to address inquiries and provide updates.
  • Database Management: Maintaining accurate records in agency systems, ensuring compliance with regulations.
  • Service Planning: Collaborating with the Case Coordination team to create initial Plans of Care based on client needs.
  • Compliance: Ensuring all onboarding processes adhere to state and agency guidelines.

Qualifications

We are seeking candidates with the following qualifications:

  • Required: High School Diploma or equivalent.
  • Preferred: Experience in a home care or healthcare intake role.
  • Strong knowledge of home care regulations and documentation requirements.
  • Proficiency in Microsoft Office (Outlook, Word, Excel).
  • Excellent organizational, time management, and multitasking skills.
  • Strong written and verbal communication abilities.
  • Bilingual: Spanish/English (other languages are a plus).
  • Must be able to reliably commute to Brooklyn for this on-site role
