families to be the solution to the seemingly insurmountable problems in their
lives, from generational cycles of poverty, abuse, and neglect to unemployment,

About the Program

At
Graham Windham our Family Foster Care programs help children to get back to
their families, adopted into a new family or connected to someone who is
willing to be an anchor for children, throughout their childhood and into
adulthood. To help us do that, we have hundreds of loving & highly
qualified foster parents primarily in Brooklyn, Manhattan, the Bronx and Westchester
County- who serve as our foster children's substitute family.
 
About the Position
The Intake Planner is responsible for the appropriate matching and safe placement of children entering regular and kinship foster homes; the replacement of children currently in foster care placements with Graham Windham; and the monitoring and maintenance of the Connections Vacancy Control and its functions. 

Duties and
Responsibilities:

  • Intake Planner must liaise with the Intake Specialist when in
    doubt of making placements
  • Utilize Solution Based Casework practice in placement discussion with ACS Office of Placement Administration (OPA) to identify and match children with the appropriate family based the needs of children. 
  • Respond to internal requests for replacement in a timely manner and utilize SBC practice on the facilitation of replacements with case planners. 
  • Assign all new referrals from OPA by using the vacancy control
    list.
  •  Maintain and share accurate
    daily foster home vacancy control list with ACS Office of Placement
    Administration.  
  • Respond to all ACS OPA after hour intake coverage rotation calls at
    night and weekends for the purpose of placement of children.
  • Prepare and distribute Intake admission form of all children accepted for placement from ACS with the case planning, medical and education teams. 
  • Complete Birth Certificates, furniture and Day Care requests as
    required by Graham's protocols and policies.
  • Contact foster parents within 24 hours after each new child is placed in a foster home. 
  • Attend all ACS Placement meetings.
  • Collaborate with the Fiscal team to ensure accurate processing of
    all new admissions.  
  • Maintain record of all OPA referral placement acceptance and
    rejection.

Qualifications    

Master's degree required with up to 5 years of foster care experience
required.Flexibility to work evenings and travel w/in the 5 boroughs and
Westchester County, extended hours as necessaryTime Management and Organizational
skills; Knowledge of MAPP and Mini MAPP, knowledge of internal and external regulations
and polices applicable to foster care and adoptionComputer literate and
familiar with CONNECTIONS and EVOLV; Bilingual Spanish/English and valid NYS driver's
license a plus .Master's Degree in Social Work, Human Services or related
field, conversational Spanish speaking with Min. 1 year Foster Care experience
preferred.

Base pay range $50,000 (BS) - $58,500/yr (Master degree)
