We are currently seeking a
Health Home Enrollment Specialist
to join our growing team. This role is essential in helping eligible patients enroll in health home and community-based programs, ensuring they receive the services and support they need.

Position Overview
The
Enrollment Specialist
plays a vital role in connecting eligible individuals to the Health Home program, ensuring they receive the medical, behavioral health, and social support services needed to improve their overall well-being. This position serves as the first point of contact for prospective members, guiding them through the enrollment process with professionalism, empathy, and cultural sensitivity.

The
Enrollment Specialist
is responsible for identifying, screening, and enrolling Medicaid recipients into the Health Home program. This includes conducting outreach, explaining program benefits, obtaining consent, and coordinating the necessary documentation. The role requires strong communication and organizational skills, as well as the ability to engage with diverse populations—including individuals with complex medical and behavioral health needs.

This is a field- and community-based position that also requires being on-site at the office five days a week. The Enrollment Specialist must be comfortable traveling across the five boroughs to meet members where they are, in addition to supporting walk-ins, office-based referrals, and team collaboration on site.

Working closely with Care Managers, providers, and community partners, the Enrollment Specialist ensures a seamless transition from referral to active participation in care management services. This position is essential in meeting program enrollment goals while maintaining compliance with state and organizational standards.

Key Responsibilities

  • Conduct patient outreach and intake for health home programs, ensuring all engagement is timely, meaningful, and progressive in nature.
  • Educate patients and families about available health home services, benefits, and eligibility requirements.
  • Document outreach, eligibility verification, and enrollment activities in the electronic care management platform (e.g., FCM, MAPP, ePACES, PSYCKES, eCW) according to BHH, DOH, and OMH standards.
  • Complete enrollment applications and ensure all documentation is accurate and compliant with regulations.
  • Verify insurance coverage, Medicaid eligibility, and required authorizations.
  • Collaborate with case managers, care coordinators, and providers to support timely service activation.
  • Maintain organized records and update electronic health record (EHR) systems.
  • Submit timely closures for candidates whose outreach period has expired or who withdraw/opt-out, in line with BHH policy.
  • Provide excellent customer service while addressing patient and family questions.
  • Track enrollment progress and report outcomes to the program manager.
  • Attend community and company events to engage with the community and conduct enrollments for eligible individuals.
  • Conduct community-based and in-home visits across all five boroughs to support member engagement and enrollment.

Qualifications

  • High school diploma or equivalent required; Associate's or Bachelor's degree in health, business, or related field preferred.
  • Prior experience in health home, healthcare enrollment, intake, or case management support strongly preferred.
  • Knowledge of Medicaid guidelines, managed care, or community health programs a plus.
  • Strong communication, interpersonal, and organizational skills.
  • Proficiency in Microsoft Office and EHR or database systems. (ePaces, Health Commerce System (MAPP), eClinical Works (eCW), PSYCKES, Foothold Care Management (FCM).
  • Ability to travel throughout the five boroughs for home and community visits via public transportation.
