Job Summary
The Care Coordination – Suicide Prevention Team Leader Coordinates the delivery of "Zero Suicide" services provided by the Care Coordination Suicide Prevention team.  Works with Crisis Stabilization Units (CSUs), inpatient psychiatric facilities, emergency departments, and domestic violence shelters to develop and implement protocols and coordination of services for eligible participants.  Ensures delivery of services in compliance with the Zero Suicide framework. Provides training to other agencies on program services and ensures protocols are followed for coordination of care. Establish follow-up and care transition protocols and tracking to help ensure patient safety, especially among high risk adults in health or behavioral health care settings who have attempted suicide or experienced a suicidal crisis, including those with serious mental illnesses and/or substance use disorder(s). Ensures collection of data and outcomes to evaluate program effectiveness. 
Responsibilities
  • Outreach and Interagency Collaboration: Leads the team in providing outreach to critical service providers (e.g., crisis stabilization units, inpatient facilities, emergency departments, domestic violence shelters), promoting awareness of Link to Life services and fostering collaborative relationships to strengthen support for high-risk individuals.
  • Protocol and Workflow Development: Establishes and refines protocols with key community partners to ensure alignment with the Zero Suicide framework, facilitating seamless transitions and continuity of care across levels of service.
  • Service Delivery and Compliance Oversight: Monitors the delivery of screenings, risk assessments, and follow-up services to ensure adherence to the Zero Suicide model and MHRC's standards for timely, effective interventions.
  • Assessment Review: Performs detailed reviews of initial and ongoing risk assessments (e.g., C-SSRS, biopsychosocial evaluations) and safety plans for all individuals enrolled in the pathway, ensuring each care plan is comprehensive, individualized, and integrates evidence-based suicide prevention interventions. Collaborates and advocates with treatment providers across departments to incorporate multidisciplinary input into the development and ongoing review of care plans, supporting cohesive and responsive client care.
  • Risk Flag Management: Oversees management of client risk flags to confirm that all individuals assessed as moderate to high risk of suicide are enrolled in the MHRC Suicide Care Management Pathway within 24 hours of a positive risk assessment. Ensures timely updates to flags and coordinates follow-up actions in line with agency protocols.
  • Safety Planning and Lethal Means Counseling: Ensures the development of individualized safety plans and the provision of lethal means counseling for all enrolled participants, with documentation updated on the same day to reflect ongoing risk management.
  • Follow-Up Protocols: Monitors adherence to structured follow-up intervals, including immediate contact within 24-48 hours of enrollment, weekly follow-ups for the first 30 days, and continued check-ins at 30, 60, 90, 120, and 180 days, adjusting as necessary based on client risk levels.
  • Missed Appointments and Crisis Response: Ensures follow-up within 8 hours of missed appointments, implementing wellness checks, emergency contact outreach, or other escalations as needed per Zero Suicide engagement standards.
  • Transition and Discharge Planning: Guides the team in developing transition and discharge plans for clients moving to lower levels of care, coordinating with other providers to ensure smooth, supportive handoffs and continuity of treatment.
  • Team Supervision: Provides direct supervision to Care Coordinators and Clinicians, holding regular individual and team meetings to review caseloads, engagement strategies, and adherence to follow-up protocols.
  • Productivity and Quality Assurance: Oversees team productivity and service quality, ensuring compliance with agency performance standards, data accuracy, and adherence to Zero Suicide practices.
  • Direct Intervention: Provides direct client services as needed, serving as a backup for Care Coordinators during crisis interventions, follow up services and urgent assessments.
  • Clinical Guidance and Case Oversight: Supports team members in complex cases by providing expertise in areas such as lethal means counseling and crisis management, ensuring high-quality, responsive care.
  • On-Call Rotation: Participates in a 24/7 on-call rotation, ensuring round-the-clock coverage for emergencies, including after-hours and weekend support for crisis situations.
  • Reporting and Documentation: Compiles and submits monthly reports on program outcomes, tracking team performance, client engagement, and compliance with follow-up schedules. Reviews documentation and billing for accuracy in alignment with agency policies.
  • Internal and External Training: Develops and conducts training for both MHRC staff and external community partners on Link to Life services, Zero Suicide protocols, and suicide risk management, strengthening interagency coordination and enhancing support for high-risk populations.
Qualifications
  • In order to be considered, Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice or a related Human Services field (a related Human Service field in on in which major course work includes the study of human behavior or development) and three years' of experience working with adults experiencing serious mental illness.  One year of supervisory experience required.

  • Ability to complete competencies assigned to position as outlined in the Orientation Competency Form.
  • Proficiency in Microsoft Office, Outlook and the use of the Internet.
  • Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.

Mental Health Resource Center, Inc. (MHRC) is a nonprofit organization that provides a wide range of behavioral health and social services to the community.
