Description: This is an opportunity for a bachelor's-degreed social worker or counselor to assist people who are challenged by mental illness and/or substance use disorder within a community of great need.

Key Duties:

  • Develop relationships with clients and become familiar with their needs
  • Know the treatment plan for each client; monitor and assist clients in following the treatment plan
  • Follow up with clients to ensure they are attending their mental health counseling, addictions counseling, psychiatric and other appointments
  • Help clients to apply for Medicaid and other benefits to which they may be entitled
  • Assist clients with applying for Medicaid and other benefits
  • Assist clients with managing their household budgets and connecting them to representative payee services
  • Assist clients with transportation as needed
  • Assess and provide referrals to appropriate resources
  • Follow up on referrals made by clinicians
  • Document services provided within the system of electronic health records on same day service is provided to include information needed for medical billing

Reports to: Department Manager

Required:

  • Bachelor's degree in social work, counseling, psychology or closely related field
  • Good oral and written communication skills
  • General knowledge of computer documentation
  • Valid driver's license

Job Type: Full-time

Pay: $20.00 per hour

Expected hours: 40 per week
