About the Role
Join Ryse Health's high-performing clinical team to improve the lives of patients with diabetes and related endocrine conditions. This hybrid role offers 2 days per week remote work, meaningful patient impact, and collaboration with a team that values innovation and autonomy.
What You'll Do
- Deliver exceptional patient care in-person and virtually, focusing on diabetes and endocrine issues.
- Conduct patient phone calls, messages, and remote data analysis to drive care decisions.
- Manage panel-level care plans for high-priority patients.
- Collaborate with physicians and the clinical team to optimize patient outcomes.
- Participate in shared call responsibilities.
- Build trust-based relationships with local primary care providers and team members.
What You Bring
- Graduate of an accredited NP or PA program with certification.
- 2+ years of clinical experience, comfortable working autonomously.
- Prior diabetes experience required; CDCES certification is a major plus.
- Current, unrestricted state license (or eligible).
- Comfortable with EMR systems.
- Passion for improving patient health and reducing the burden of chronic disease.
