Home-Based Nurse Practitioner – Locums Program (Michigan)
Location: Michigan (primary launch area: Kent County; additional statewide coverage as needed)
Start Date: October 15, 2025
Initial Term: Through December 2025 (extensions likely)
Schedule: 5 days per week, 8-hour days within 8 AM–8 PM window
Position Overview
We are seeking Nurse Practitioners to join a Home-Based Locums Program in Michigan. This innovative program focuses on providing high-quality, in-home care to health plan members—primarily seniors (65+) as well as adult Medicaid recipients (18+).
Nurse Practitioners in this role will conduct in-home visits to support preventive care, close quality gaps, and perform comprehensive wellness assessments. The program emphasizes patient-centered care, social determinant assessments, and collaboration with care coordination teams.
Key Responsibilities
- Conduct 5–6 member visits per day (up to 8 scheduled to account for cancellations).
- Provide comprehensive in-home assessments, including:
- Annual wellness visits (telehealth limited for locums).
- Level 2–3 diagnostic visits (A1c, bone density, KED, FIT, retinal exams, etc.).
- Level 4 comprehensive annuals (full wellness, labs/diagnostics, SDoH evaluation).
- Document visits and follow-up recommendations using iPad/EHR.
- Collaborate with Care Guides who create individualized 30-day care plans for members.
- Maintain safety protocols—if unsafe, leave and notify coordinator.
- Work independently in the field with support from clinical trainers, practice coordinators, and leadership.
Schedule & Work Model
- Hours: 8-hour day (includes drive time + charting).
- Workdays: 5 days per week — preference for M–F, but flexible options available (e.g., W–Sun).
- Holiday Weeks: No visits on Thu/Fri/Sat/Sun of major holiday weeks.
- Independence: Providers work solo; member check-in handled by support staff.
Requirements
- Licensure: Active Michigan NP license + active MI RN license (standard MI requirement).
- Certification: Board certified FNP-C or GNP (Family Practice, Internal Medicine, or Geriatrics).
- Enrollment: Medicare & Medicaid credentialed in Michigan (via CHAMPS).
- Experience:
- New grads considered if prior RN experience is strong.
- Preference for in-home care experience, diagnostic screenings, and 1:1 patient interaction.
- Skills:
- Comfortable with technology (iPad-based documentation).
- Experience with screening/diagnostic workflows (HEDIS, HRAs, risk adjustment evaluations).
- Not Accepted: Physician Assistants (PAs).
Coverage Areas
Primary assignments begin in Kent County, MI with expansion across multiple regions. Providers must be able to cover:
- Detroit Metro: Oakland, Wayne, Macomb
- East Central MI: Saginaw, Midland, Bay, Clare, Gratiot, Gladwin, Arenac
- Southeast MI: Monroe, Jackson, Ingham, Calhoun, Hillsdale, Washtenaw, Livingston, Lenawee
- The Thumb: St. Clair, Genesee, Lapeer, Tuscola, Sanilac, Shiawassee, Huron
- Other regions: Kalamazoo, Grand Traverse, Barry, Berrien, Alpena, Emmet, Manistee, Cheboygan, etc.
Local Kent County providers may be considered for Kent-only coverage.
Compensation & Logistics
- Daily rate (based on 8-hour day; guaranteed even with cancellations).
- Paid training modules prior to start ($50/hour).
- Travel & housing covered: flights, hotel, rental car or mileage reimbursement.
- Medical equipment provided (scrubs/shoes not included).
Best fit for Nurse Practitioners who are independent, flexible, and passionate about providing preventive and diagnostic care directly in patients' homes.
