Background

The Center for Human Development has an innovative partnership with Contra Costa Health Services to support the needs of patients who are at risk of health disparities. To prevent disparate health outcomes for Latino, African American, prison reentrants and low income populations with chronic disease, CHD provides: trained "Promotoras" (Spanish language health promoters), African American Health Conductors, Reentry Health Conductors (for formerly incarcerated) and Patient Navigators, to support low income populations with their management of chronic disease as well as linkage to health benefit and social need support. The staff works under the Reducing

Health Disparities Unit, Contra Costa Regional Medical and Health Centers, Contra Costa Health Services. Their work is based at one of the ten health centers operated by Contra Costa Regional Medical.

Job Description

The Promotora Health Navigator will be part of the Contra Costa Regional Medical Hospital and Health Centers' Promotora Team. This person will provide hands-on assistance to community residents to enroll in State and County health benefit programs, link patients and residents to community health resources, and provide health navigation assistance and support to cancer patients. They serve as a bridge between Spanish-speaking immigrants and Contra Costa Regional Medical Hospital and Health Centers.

Duties & Responsibilities o Assists community residents and patients in accessing county and community health services through help in making medical, dental, or mental health appointments at various health centers o Educates the community and provide hands on enrollment assistance on Medi-Cal, and other county and community health benefit programs through a computer based e-source linkage tool called REACH at various health center sites. o Provides health navigation assistance to Contra Costa Regional Medical Center cancer patients, if assigned. Duties may include: providing reassurance to infusion patients, health navigation, linking patients to resources, assisting patients with forms and applications, working with the Oncology FNP and social worker o Conducts community outreach to promote Medi-Cal, and other County/State health benefit and resource programs. o Participate in team meetings as well as staff meetings at the Center for Human Development. o Supports the Reducing Health Disparity Manager with special projects o Other duties as assigned and relocation as needed.

Knowledge, Skills, and Abilities: o Ability to fluently read, speak and write Spanish o Knowledge of community health issues. o Familiarity with Contra Costa County. o Active listening skills. o Ability to express ideas, suggestions, and needs in a respectful and clear manner. o Confidence in facilitating health education information and activities with groups of patients. o Ability to organize and set priorities and manage multiple tasks. o Ability to work independently and as part of a team. o Demonstrated maturity and reliability. o Ability to recruit and motivate adults and children. o Demonstrated skills in coordinating and facilitating groups of adults and children. o Demonstrated ability to establish and maintain effective working relationships with the public, co-workers, supervisors, and clinic staff. o Ability to adjust time to allow for some evening or weekend meetings.

Education, Credentials and Experience:

o Associate degree in community organizing, health, human services, or related field and/or Community Work experience. Excellent position for an individual pursing a degree in health education, public health, community organizing, or other related fields. o Two to three years of experience working with targeted population. o Computer proficiency sufficient to: utilize resource linkage software application, schedule patient visits, input data, create reports and flyers, and receive and respond to emails.

Special Requirements:

o Occasional evenings and weekends as requested;

o Valid California Drivers License (Required)

o Reliable transportation and proof of insurance (Required)

o Successful background check (Required)

o Clear T.B. test. (Required)

Starting Salary:

$23.75 hourly rate

Benefits:

Benefits include medical, dental, vision, 12 holidays and 19 vacation days per year and a retirement plan. This is a full-time (40 hours per week), non-exempt position.
