Overview

We are seeking a compassionate and skilled Bi-Lingual Lead Care Manager to join our dynamic team. The ideal candidate will have extensive experience in client care and connecting client's to resources to support the social determinants of health. This is a vital role that requires a high level of dedication, attention to detail, and the ability to manage multiple responsibilities simultaneously.

Duties

  • Conduct discharge planning to ensure seamless transitions for patients.
  • Collaborate with multidisciplinary teams to provide high-quality patient care.
  • Develop and implement case management strategies to optimize patient outcomes.
  • Coordinate with physicians and other healthcare professionals to ensure patients receive the necessary treatments.
  • Maintain a thorough understanding of program policies and procedures.
  • Deliver compassionate services based on best practices in the recuperative care field.
  • Monitor client progress and adjust care plans accordingly.
  • Ensure that all care plans align with HIPAA regulations.
  • Perform diagnostic evaluations and order necessary tests and treatments.
  • Manage patient flow and prioritize care tasks effectively.
  • Provide patient education and support to promote self-care and independence.
  • Maintain accurate and detailed records of patient care and outcomes.

Requirements

  • A bachelor's degree in a healthcare or human services related field.

-Previous Enhanced Care Management experience preferred.

- Experience working in a hospital and/or shelter setting.

- Proven experience in discharge planning and case management.

- Experience with inputting data into Electronic Medical Records (Nextgen/Chorus)

- Knowledge of HIPAA regulations and hospital policies.

- The ability to work effectively in a fast-paced environment.

- Excellent communication and interpersonal skills.

- A commitment to delivering high-quality client care and promoting client safety.

- Experience working with Managed Care Plans with submitting authorizations and/or referrals.

Job Type: Full-time
