FVK is recruiting for a hospitality focused Admissions Nurse/Outreach to join our team This position is responsible for managing the Health Center admissions process and for establishing and maintaining an open, effective communication between the facility and referral sources, physicians, and third-party payers.  This person will develop and implement marketing and advertising campaigns; track admissions/discharges/referral data; maintain promotional materials inventory; plan meetings and trade shows; maintain databases and prepare reports.

Here are a few of the daily responsibilities of the Admissions/Outreach:

  • Obtain referrals from hospital social workers, physicians, or family members keeping wait list current.
  • Functions as the Health Center & Rehabilitation Center's public relations person, providing tours of the facility and pertinent information regarding admissions as well as occasional off-site visits to hospitals and doctor's offices.
  • Obtains admission documents from the discharge planner, resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information and medical information, attending physician information and other pertinent and required documentation prior to admission.
  • Maintains monthly referral statistics and reports to QAPI committee.
  • Reviews and completes all admission related paperwork with the resident and/or responsible party including Medicare Part A coverage and non-coverage procedures.  

Here are a few of the qualifications we need you to have:

  • RN License preferred.  
  • Experienced in long-term care and service delivery.
  • Good working knowledge of Medicare Part A and certification procedures
