Job Summary and Qualifications

The Navigation Program Coordinator is responsible for the development and coordination of designated department activities and operations initiatives to achieve program goals. The Navigation Program Coordinator collaborates with Program leadership to assist with developing and coordinating comprehensive navigation programs, processes, and toolkits across the enterprise.

What you will do in this role:

  • Supports program initiatives by developing collateral, presentations and communications in collaboration with strategic and operational leadership
  • Ensures toolkits and program assets stay current and continuously manages enhancements
  • Collaborates with all appropriate parties to develop efficient processes that meet the needs of the business
  • Executes the details of assigned strategic projects
  • Works alongside Navigation Programs team to ensure communication and deployment strategy are followed through
  • Optimize the continuum of care and support the creation of programs that will reduce the risk of patient outmigration
  • Collaborate with the strategic and operational leadership team, enterprise navigators, and service lines to develop and optimize programs and tools
  • Collaborates with the Parallon and ITG teams to create online tools and resources for the operations team
  • Demonstrated ability to work effectively with other disciplines and departments.
  • Must be flexible to meet work demands
  • Must have excellent written and oral communication skills
  • Develop systems to collect data for analytics, as needed, to validate progress

Qualifications:

  • High School Diploma (GED) or Vocational/Technical School required
  • Associate's degree program preferred
  • Bachelor's degree program preferred
  • Three to five years navigation operations or program coordination experience
  • Healthcare experience is preferred
