Position Summary:

Manages multi-disciplinary team of Care Coordinators and Benefits Specialists to ensure Housing First supportive housing principles and best practices are met. The Care Coordinator Manager oversees the implementation of services and programs that work together to aid residents in achieving their maximum potential.

Essential Job Functions:

  • Provide oversight of case management and care coordination for Towne Twin Village residents through collaboration and consultation within other complimentary departments .

  • Coordinate closely and directly with Housing Coordinator team to assure cooperation for resident assistance, move-in, troubleshooting, resident services and support.

  • Direct and oversee Care Coordinator departmental objectives including communication, processes, reporting, measurement and quality assurance check-points.

  • In conjunction with Director of Support Services, refine resident services plans including protocols, standards, training and regular measurement of standards.

  • Manage program evaluation data, quarterly board and resident council reports, and data quality related to care coordination.

  • Administer system and security for Housing Management Information reporting.

  • Prepare compliance documentation for various funding sources and compliance organizations.

  • Hire, train, manage and coach Care Coordination and Benefit Specialist team.

  • Oversee and direct the build-out of Benefit Navigator for SSI/SSDI outreach and access.

  • Performs related duties and fulfills responsibilities as required.

  • Regular and reliable attendance is an essential job function.

Job Requirements:

  • Excellent interpersonal and written communication skills.

  • Knowledge of social work theories and Code of Ethics.

  • Open attitude to adopting a new case management approach.

  • Previous management experience a plus.

Knowledge, Skills and Abilities:

  • LMSW preferred.

  • Bilingual preferred.

  • Experience with housing and/or homelessness

  • Ability to collect, analyze and understand data trends

  • Ability to communicate clearly and effectively.

  • Ability to conduct assessments, intake, evaluation, implementation of care plan and service planning.

  • Ability to understand the organization and operations of Towne Twin Village and of partner agencies as necessary to assume assigned responsibilities.

  • Ability to work independently in the absence of supervision.

Physical Requirements:

  • Able to sit and work at a computer for extended periods of time.

  • Able to stand and walk for extended periods of time, either indoor or outdoor.

  • Able to stoop, kneel, bend at the waist, and reach on a daily basis and lift up to 20 lbs occasionally.

  • Able to perform general office administrative activities.

  • The mental work demands include regular attendance, working well with others, verbal contact with others.

Pay: $58, $67,000.00 per year
