Outreach Coordinator for Psychiatric Patients

As an Outreach Coordinator for Psychiatric Patients, you will provide vital support to the care coordination team in delivering comprehensive outreach care services to individuals with psychiatric conditions transitioning to the community. You will assist in coordinating, facilitate locating and the delivery of psychiatric care in the community. Your responsibility also includes ensuring the smooth operation of care transition, and supporting patients in navigating the community healthcare system. Your responsibilities may include:
Patient Support: Provide direct support to patients with psychiatric conditions, assisting them in accessing and navigating mental health services. Offer emotional support, actively listen to their concerns, and provide information about available resources.
Care Coordination Assistance: Assist the care coordination team in initiating admission requirements as needed. Help gather patient information, update records, and ensure all patient information is accurate, up-to-date, and aligned with patients' needs and goals.
Appointment Scheduling: Coordinate and schedule appointments for patients with psychiatrists, therapists, and other healthcare providers. Ensure that patients are aware of their appointments and have the necessary information to attend them.
Resource Referral: Identify and connect patients with appropriate community resources, support services, and psychiatric rehabilitation programs. Assist patients in accessing mental health resources, including counseling, support groups, and crisis intervention services.
Documentation and Record-Keeping: Maintain accurate and organized patient records, documenting appointments, and other relevant information. Adhere to legal and ethical guidelines regarding patient confidentiality and data protection.
Communication and Collaboration: Facilitate effective communication and collaboration between patients, healthcare providers, and other stakeholders involved in the care process. Relay important information, coordinate care transitions, and ensure that all parties are informed and engaged.
Administrative Tasks: Assist with administrative tasks such as data entry, filing, and organizing paperwork. Help maintain inventory of supplies and ensure that necessary materials are readily available.
Qualifications:
Bachelor's Degree Social Work, Psychology, Biopsychosocial, Nursing, (RN or LVN) or a related field preferred
Reliable transportation and willingness to travel within the designated city and its surrounding communities.
Knowledge of psychiatric disorders, treatment modalities, and evidence-based practices.
Strong communication and interpersonal skills to effectively engage with patients, families, and healthcare professionals.
Ability to assess and manage psychiatric crises and emergencies.
Familiarity with community resources and support services for individuals with psychiatric conditions.
Excellent organizational and time management skills to handle multiple cases and prioritize tasks.
Proficiency in documentation and record-keeping.
Empathy, compassion, and a non-judgmental approach to patient care.
Knowledge of relevant legal and ethical guidelines, including patient confidentiality and privacy regulation
Basic knowledge of psychiatric disorders, treatment modalities, and mental health resources.
Strong organizational and time management skills to handle multiple tasks and prioritize effectively.
Excellent communication and interpersonal skills to interact with patients, families, and healthcare professionals.
Empathy, compassion, and a non-judgmental approach to patient care.
Attention to detail and accuracy in documentation and record-keeping.
Ability to work collaboratively in a team environment.
Familiarity with medical terminology and electronic health records (EHR) systems is a plus.
