Summary
The TLP Outreach Coordinator is responsible for identifying, engaging, and building trust with runaway and homeless youth eligible for the Transitional Living Program. This position connects youth to safe housing and essential services through street-based outreach, relationship-building, and coordination with internal teams and community partners. The Outreach Coordinator plays a critical role in ensuring youth have access to stable support systems and pathways toward long-term independence.
- Provide youth engagement utilizing New Day's primary approaches which include: the Nurtured Heart Approach, Trauma Informed Care, and Positive Youth Development.
- Provide services in a culturally responsive and youth-friendly manner, including enrollment and comprehensive needs assessment.
- Utilize effective, positive communication skills with peers, staff, families, community and the public and embody New Day's ten core values in all interactions.
- Coordinate closely with internal staff, including the Housing Coordinator and Housing Specialist, to ensure smooth referrals, case planning, and service delivery.
- Document all outreach and aftercare provided, or attempts made.
- Track data and develop reports as requested including a weekly update email to the TLP Program Manager.
- Follow all personnel and agency policies and procedures.
- Participate in team meetings, supervision, and training to support continuous learning and alignment with program goals.
- Perform other duties as assigned.
- OUTREACH
- Maintain consistent communication and collaboration with community partners, including other outreach teams, shelters, and service providers, to ensure coordinated care.
- Able to be in unpredictable situations and assess for risk.
- Conduct regular street-based outreach to locate and engage runaway and homeless youth, using trauma-informed and youth-centered approaches.
- Build trusting relationships with youth and assess their needs to determine appropriate services and eligibility for TLP housing.
- Plan and execute regular outreach events in partnership with other community members; create flyers and outreach materials for these events including the New Day outreach kit (hygiene pack)
- Provide basic needs support, transportation assistance, and crisis intervention as needed to help stabilize youth and connect them to longer-term services.
- Participate in weekly walking and pop-up street outreach activities. Facilitate outreach plans when requested by Housing Director
- Participate in community street outreach efforts; create new outreach opportunities for New Day.
- Maintain accurate and timely documentation in the RHY-HMIS system and internal databases, tracking outreach efforts, referrals, and youth outcomes.
- Support youth to access shelters and housing.
- Transport youth to shelters, appointments and accessing resources as needed.
- Qualification
- A bachelor's degree in social work, psychology, human services, or a related field is preferred. However, equivalent work experience will be considered in lieu of a degree.
- Two years of experience working directly with youth, particularly those experiencing homelessness, involvement in the child welfare or juvenile justice systems, or other systems of care.
- Experience in street outreach, crisis intervention, case management, or similar community-based work is strongly desired.
