Case Manager
- Organizing, facilitating, and evaluating care options for individuals' mental, physical, and emotional health.
- Managing the care of individuals with medical, social, and educational needs.
- Coordinating resources and services, monitoring progress, and advocating for clients.
- Assisting clients in locating suitable facilities and evaluating treatment alternatives.
- Gathering information, developing strategies, and showing concern for clients and their families.
- In this position, you will partner with other certified clinical staff to provide high-quality care to a minimum of 10 clients per day in both individual and group sessions.
- Weekly audit of live members list ( Molina Kaiser IEHP)
- Auditing CHW folders
- Conduction weekly meetings
- Collection of paperwork, Auditing paperwork
- Responsible for maintaining and fostering relations with community partners and professional contacts
- Firm knowledge in computer softwares * excel, Word,
- Detailed 'Calendar orientated
- Able to gather resources
- Weekly Chw review meetings individually
- Submitting referrals and authorizations to providers
- Weekly HIPAA Compliance check with clients and office
- Training all new CHW and community specialist
- Call list compliance
- Inventory check ins making sure all equipment is working properly
