Role Summary

The Case Manager at SOMACC's Family Resource Center is responsible for providing comprehensive, strengths-based case management to families, including intake, assessment, service planning, and community referrals. Key duties involve conducting outreach, facilitating parent education groups, providing crisis response, and maintaining collaboration with other providers.

As our Case Manager, you are the navigator, advocate, and chief problem-solver for families facing challenges. You'll ensure every family receives the support they need to thrive.

Position Responsibilities

  • Coordinate services to include intake, assessment, and referrals to internal services and community service providers. Intakes, assessments and service plans are done collaboratively with families.
  • Provide outreach to community organizations and neighborhood residents to share information about services and engage families in case management services.
  • Conduct home visits regularly as requested by clients.
  • Participate in joint visits and transitional meetings with CPS workers within the time frame protocol as needed.
  • Conduct comprehensive assessments using the Family Assessment Matrix tool and Family Service plan.
  • Provide crisis response and review safety plans regularly.
  • Maintain collaborative working relationships with other providers involved in the family's case (including HSA, social workers, and other community organizations), while supporting the family.
  • Maintain required documentation for funding and statistical compliance.
  • Demonstrate knowledge of the resources in the community.
  • Participate in agency, provider and community meetings as directed, including weekly team meetings and monthly and/or quarterly workgroup meetings.
  • Coordinate with the FRC Director and receive supervision on a weekly basis.
  • Facilitate parent education classes, parent-child interactive groups, workshops, and support groups for FRC clients.
  • Create curriculum for parenting groups and workshops.
  • Perform other duties as may be assigned.

Site Responsibilities

  • Provides comprehensive information and referrals to clients, as needed.
  • Works collaboratively with the Family Resource Center Director, Early Care and Education Advocate, Parent Education Coordinator, and Family Advocate in order to provide optimal services to the community.
  • Responsible for providing a welcoming environment, assisting drop-in clients, and facilitating the intake process for community members.

Physical Demands

  • Ability to perform duties while standing, sitting and/or walking.
  • Ability to use hands for simple grasping and fine manipulations.
  • Ability to bend, squat, balance, reach.
  • Ability to lift, carry, push or pull medium weights, up to 40 pounds.
  • Ability to communicate effectively using speech, vision and hearing.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling, or related field or equivalent years' experience working in a similar position.
  • Experience in providing case management, support and advocacy services, information and referral to different services and resources as needed.
  • Experience in client interviewing, assessment, and service plan development.
  • Committed to utilizing a strengths-based, trauma-informed approach to working with children and families.
  • Sensitivity to the cultural and socioeconomic background of the children and families being served.
  • Interest in a collaborative work environment and partnering with a team of service providers.
  • Willingness to travel within San Francisco for programs and events.
  • Can work flexible hours that include evenings and weekends as necessary.
  • Upon hire must be able to pass a physical exam, T.B. screening, and provide vaccination records for measles, pertussis and flu vaccines (required).
  • Upon hire, must pass a Department of Justice fingerprint clearance and a Child Abuse Index clearance as required by licensing (required).
  • Bilingual English/Spanish, English/Tagalog, or English/Chinese (reading, writing and speaking. is a plus.
  • CPR and First Aid Certificate preferred.
